You might have to remove the radio or other dashboard parts in order to check if the duct feeding that vent is still attached to the plenum.
It might be possible to view this area by removing the air bag cover---if you van didn't come with factory air bag this would be super simple---just pop that cover off. If the air bag is in place you might be able to remove the right side lower trim panel and look upward. Removing the dog house at this same time might also give a better view of all this.
