ive done them with a lift, and if you remove the starter, and take out the motor mount bolt and with a block of wood jack the motor up just a little to roll it, it will give you plenty of room to slide it out towards the back of the truck....it might be a horrible pain on the ground but im sure it could be done

OK, I finally tore into the Pass side manifold. None of the bolts were broken, all the nuts came off with heat. I was ablet to get the manifold out without pulling the starter. The manifold was cracked.
I'm doing this on the ground and so far so good.
I need to change out the studs and start reassembly.
